Optimising your medications

A consultant pharmacist can work with you to ensure that you are getting the most out of your medications. The pharmacist may identify a medication that is no longer benefiting you and can help you and your doctor to safely stop it.

 

Visiting your home

The pharmacist will arrange an appointment to visit your home at a time that suits you best. We are based in the South Eastern suburbs of Melbourne.

The appointment can be conducted in English and Greek. Phone interpreters are also available.

There is no cost to you for this service. The pharmacist will be reimbursed by Medicare.

To get started, ask your doctor if a Home Medicine Review could be helpful for you. General practitioners and specialists are able to make referrals.

Frequently asked questions

  • A Home Medicine Review does not cost you anything if you have a Medicare or DVA card. You may need to pay to see the GP to get the referral.

  • Yes, you will need a referral from a doctor. This can be from your GP or from certain hospital-based doctors.

  • The review usually takes about 45 minutes. This allows plenty of time for discussion and questions in the privacy of your own home without feeling rushed or being interrupted.

  • It is very helpful for carers to be involved in the review. You will have the opportunity to ask questions and discuss any concerns that you have. You will also get information to help you manage the medications of the person you care for.

  • Yes, a copy of the report can be provided to you and your carer.
